Description : Under this scheme, eligible persons with disabilities are entitled to receive a monthly assistance of Rs. 1000/-. The State Government provides Rs. 700/- of this amount, while the Central Government provides Rs. 300/-Eligibility : State of Residence = Gujarat Age = 18 to 79 Should have a BPL score card rated 0-20 Should have disability of more than 80%Process : Offline 1. To apply immediately in rural areas visit your Village Computer Entrepreneur (VCE) and in urban areas visit District Social Security Officer Office. Also visit your Gram Panchayat/Municipality office. Get form from there and submit the same at the same office with required documents Online 1. Online applications can be made at E-Gram Kendras under Digital Seva Setu or from Digital Gujarat Portal. 2. Visit https://www.digitalgujarat.gov.in/CitizenApp/Citizen/CitizenWEBUI/Registration.aspx, register with mobile number and email ID. Generate user ID and password. 3. Login, select scheme and apply.
