Description : Pradhan Mantri Jan Arogya Yojana (PM-JAY) is implemented to reduce the financial burden on poor and vulnerable groups arising out of catastrophic hospital episodes and ensure their access to quality health services.Eligibility : A resident of Maharashtra. Citizen’s name should appear on NHA beneficiary portal Must have ration cardProcess : Offline:

1. A family should approach their nearest Civil hospital/Primary Health Care Center in urban areas or ASHA worker/Primary Health Care Center in rural areas to check if they are eligible for Ayushman Bharat Yojana as per the SECC list.
2. If the family’s name is in the list, they can apply for a Ayushman Bharat card with the required documents.

Online:

1. Visit Ayushman Bharat beneficiary portal: https://beneficiary.nha.gov.in/ or Ayushman Bharat app.
2. Log in using mobile based OTP.
3. Search your name on the portal using Family ID(ration card no.)/Aadhar Card or beneficiary name
4. Complete Aadhar based eKYC for all the family members whose status is ídentified.
5. If the matching score is more than 80%, then the card will be generated in a week.
6. Download the card using Aadhar based OTPBenefit : Benefit cover of ₹ 5 lakhs per family per year (on a family floater basis)
